Rear wheel

No, I torqued it correctly back in 2014. I don't remember the torque, but I remember looking it up when I changed to the Bridgestone tire.

I just put my rear wheel back on last weekend , 1st time doing it myself so now this has got me a bit paranoid , does the spacer require moly lube on the outer side ?

81 foot pounds if I 'member correctly.
I've had my rear wheel off so darn many times in the last 4 years, I'm running out of axle Grease!

Ask a local machine shop if they can sleeve it. Might get it done for a couple hundred. Done correctly with a steel sleeve it will last longer than a new wheel. Do so only if the rest of the wheel is undamaged.
That sounds legit

thanks for pic.
that is what happens when the bearing seizes up.
i do not think that u messed up.
how many miles on your bike?

Agreed. I'm a machinist. Should be a quick and inexpensive repair, unless a wheel is available cheap. If you lived closer I'd gladly help you.

if u tighten the axle nut first then tighten the final drive that would put a lot of pressure on the bearings and make them go bad.
