It is a factory design Classic or Roadster screen from Triumph with an aftermarket Clearview (USA made) lipped screen with the adjustable vent. Clearview make larger screens that fit the Triumph mounting hardware but do not sell or supply hardware. I have a few Triumph factory accessory Classic and Roadster screens and brackets in my shed and have sold them around the world - shipping from Oz with screen is about Au$150.
Au$350 just screen and chrome brackets but you need the Triumph fork fixing kit that is used for all the Triumph factory accessory screen versions for dual headlight Rockets including the 'Flyscreen', clear 'Sport' screen (fly screen size), 'Classic' 5 bolt TT chrome strap, 'Roadster' 5 bolt TT chrome strap and rare 'Summer' 3 bolt TT chrome strap.
I have an excellent used screen with a used fork fixing kit for $Au450 + postage.
The fork fixing kit (4 fork clamps for attaching screens) is no longer available from Triumph either but a knockoff is made by a Russian eBay seller but no longer available due to the war.
I have a just three new old stock sets left of the fork fixing kit for Au$150 each.

I actually have my own Clearview screen with the vent using the 3 bolt Summer Chrome TTT strap and Memphis Shades extra top and side extensions from the USA. tested with a huge load up to 120mph. This complete unit with the stainless steel side blades all detaches or attaches with the same 4 bolts as any other Triumph accessory screen in about 5 minutes. I made some brackets up to attach the blades to the factory brackets so it all comes off or on together.
The vents can be bought off eBay from China for Au$47 (about £24.23 for you without 10% GST) free shipping and can be installed in any screen.

I have a set of the Triumph brackets without the screen here too if you just want them to get a USA made Clearview too. Au$300 and post would be about AU$100 at a guess. A used set of Fork clamps will cost another Au$100. The Clearview screen will probably cost about Au$300-$400 to get to UK so if I was you I would buy a stock Triumph screen and add a vent or top-lip too if you want/need.

Your photo shows a Triumph factory accessory Matt Black with White stripe Flyscreen but now mostly 'Out of stock' from Triumph. To have it fitted you MUST already have the Fork Fixing kit with the 4 clamps chrome from factory but many have powder-coated them black too. A custom fiberglass version is made in the UK by Custom Triumph Parts who sells direct on FB as Fibreman in Rocket III Groups or via an eBay site.

Yes agreed about the fork fixing kit but in addition, I would still need the two parts mentioned above to mount the clearview windscreen in my opinion.

Yes, correct part numbers BUT you still need the chrome TT straps and bolts either 3 or 5 bolt.
The straps are not and never were sold separately by Triumph and only sold as part of the 'Screen Assembly', either:

The Triumph Parts diagrams are wrong on some items, but the part numbers are still correct. The image used for the 5 bolt Roadster screen is actually the Summer 3 bolt screen and this mistake is repeated worldwide in Triumph parts listings.
Clearview does both 3 and 5 bolt screens for Triumph Rockets on request.
